Remove obsolete md4c patch

This was added in 1591a959876bc5a37aacc817e637400c72bf6623 on top of an
older version of md4c, but those casts now exist in upstream code,
which we upgraded to in c29a235833410fde4cb4d502f89129bccd7403f0
